A 16- session program was develped, which was offered to bipolar patients and their significant other. This program coveted historical aspects including the current biological understanding. Evidenced based psycho pharmacology intervention to date are discussed together with side effect profikes and aspects of pharmacokinetics and dynamics, the monitoring and recognition of inter episodal supsyndromal symptoms including demorali zation, substance use and abuse , treatment collaboration as opposed to treatment adherence and relapse prevention options with legal responsibilities and avenues including the Mental Health Act are in the curriculum. Finding meaning in set backs, using creative creative values: attitudinal values and experiential values are discussed, The consideration of the bipolar patients loss of insight during the episode and thus the need for the patients loss of insight during the episode and thus the need for the significant other (Surrogate Ego) to step in with an agreed management plan being drawn up. This forms the basis of assessing and achieving graduation from the program. The principals of empowerment and excellent illness management skills including responsibilities are instilled. Aim: To study the efficacy and effectiveness of this intervention on improving global outcomes for bipolar patients. Method: 60 bipolar patients who are in the maintenance phase of the illness and being managed following an acute episode are randomly allocated to two group. First group the contorl gets routine case management and equal clinical contacts while the second group took part in SIMSEP-BD offered by the SIMSEP Facility- Senior Psychologist, Senior nurse and a Senior social worker under the guidance and supervision of a psychiatrist. A research officer tracks outcomes measures for 12 months, 15 months and 18 months including functional scale questionnaire and readmission. Results: At 12 months point there was significant positive outcomes in the intervention group over control group. Statistical analysis including survival analysis showed superiority of intervention over control case management group. This significance was maintained at 15 months and 18 months. The areas of significance were in treatment adherence, and collaboration, experience of adverse events, readmission, dropouts and the scale of psychological functioning the functional scale questionnaire. Conclusion: This model of the use of Systematic Illness Management Skills Enhancement Program in Bipolar Disorders for patients with bipolar disorder deals with demoralization-the impact of the insult of the illness on the person, and can enhance trust, which is fundamental of a therapeutic alliance ensuring treatment adherence and improve outcomes. The inclusion of the significant other-the surrogate ego for times when insight is lost in the patient, together with the empowerment that this model of care offers suggested overall there were improved longitudinal outcomes, thusresulting in improved quality of life and a significant positive impact on the health economics for the service, the bipolar patient and the family.
