Community Psychiatry is a term like rehabilitation that means many things in many different countries. Community care in many developed countries. Is motivated by the need to reduce mental hospital or mental wards beds to a minimum while enhancing the patient’s life in this own home. In developing countries the community care is often meant to provide a basic service for those who default treatment, are in remote places or remain undetected and sometimes abused due to ignorance. Brunei is a prosperous oil rich country of 360,00 people and 360 doctors of whom 12 are psychiatrists. There are 2 psychiatric inpatient units with less than 50 beds in all. The country has no mental hospital but instead has 3-day rehabilitation centers and 4 outreach psychiatric clinics. Despite these in a country that is 125 kms by 60 kms, there are several areas that remain uncovered by the fairly good mental health services. The 1997 inauguration of mental health community team has enhanced care for the mentally ill who are normally not covered by the clinic and inpatient services or the day rehabilitation services. There are however problems of staffing, transport and frequent crises that make running of the community services difficult, so that on two occasions in the past 10 years. The community service however has support from families of the mentally ill although they are not organized in a formal association. This paper describes the paradigm of a Community team in Brunei and is illustrated with pictures of the work of the team.
