The Hamilton Rating Scale for Depression, the most widely used rating scale for depression, still has never been translated into Thai. We developed the Thai version of this scale and tested the validity and reliability in Thai patients. Method We translated the Hamilton Rating Scale for Depression into Thai. The back translation by bilingual psychiatrist was done. After the interrater reliability was satisfied, we used this scale with 50 depressive patients. Global Assessment Scale was used as a gold-standard instrument. Statistical analysis was performed to test the validity and internal consistency of the scale. Results The kappa value of the scale was 0.87. The Spearman's correlation coefficient which indicated the validity was -0.8239 (P ‹0.0001). The internal consistency was good (standardized Cronbach's alpha coefficient = 0.7380). Results of factor analytic studied revealed that our patients’ depressive syndrome pointed toward anxious-depression mixture. Some symptoms, such as poor insight, hypochondriasis, agitation and guilt feeling, yielded low correlation with the total score. This finding might correlated with cultural factors. Conclusion The result revealed that the Thai version of Hamilton Rating Scale for Depression (Thai HRSD) has satisfied validity and reliability. So this rating scale can be used to measure severity of depression in Thai patients.
