The Early Psychosis Intervention Programme (EPIP),Singapore was initiated in 2001 to encourage earlier effective help-seeking, and the provision of a holistic and comprehensive treatment of the first psychotic episode. As this program was supported by the Ministry of Health of Singapore, a set of pre-determined indicators were agreed upon: process indicators such as the development of evidence-based clinical pathways, the establishment of care management, and coming up with a strategy for public education; outcome indicators such as the number of individuals screened and accepted into the program, the change in EPIP’s referral patterns, and a reduction in the duration of untreated psychosis. The data showed a reduction of the median duration of untreated psychosis from 12 months to 4 months with a positive shift in the referral pattern towards self, family, and primary care referrals. We also found that patients were given better quality of care, including appropriate use of medications, sustained reduction in the severity of the psychosis, improved functioning, which translated to high levels of service satisfaction reported by our patients. Based on this initial 5-year performance report, EPIP is now an integral part of the National Mental Health Blueprint and is now recognized as standard treatment of care for first-episode psychosis. Moving ahead, the plan is to further enhance the components of indicated prevention and early detection within the programme.
