Objective: To develop a depression surveillance system in Thailand for both general and at risk people, including prevention of depression relapse at provincial level. Methods: In a research and development, pilot study, 946 public health volunteers, 328 nurses, 21 residents in primary care and 297,588 peoples in a community of Yasothorn province. The first step was situational analysis of health care system,review literature for technology, measurement and information of surveillance system and then implementation, evaluation and expansion. Results: Based on the situational analysis, it was found that there has not been any depression surveillance system, particularly one, which is contextualized to the circumstances. Further more there has not been any screening and diagnostic test, as well as prevention of such symptom. Accordingly, a contextualized depression surveillance system was developed. The system consisted of two components namely 2-question screening test (2Q) used by public healtgh volunteers, and 9-question diagnostic test (9Q) and 8-question diagnosis for suicide (8Q) used by public health officers. Apart from these tools, at risk people also received supports according to the severity of the symptoms including health education, counseling, psychosocial intervention, and pharmacotherapy. Information technology, which was networked to local databased, was also implemented to prevent further development of the symptoms. Among 297,588 people in the target group, 9.89% were at risk of having depression. When diagnosed with 9Q, 7.20% of them were found to suffer from depression, with 71.27% mild, 18.95% moderate, and 9.78% severe symptoms. Surprisingly, it was found that all people diagnosed as patients were at risk for suicide as well. Nevertheless, 31.94% of them received supports as guideline; of this 0.95% relapsed. What seemed to be significant was that the rate of access to the system increased for 155.84%. Conclusion: For the first time in Thailand, the depression surveillance system, developed and piloted at Yasothorn province, became effective. The system could find, screen, diagnose, support and treat the patients holistically. More importantly, the system was contextualized to the needs of local people, socio-cultural context, and resources available at local care units. Based on this finding, it is hoped that the situation concerning GBD and DALY in Thailand will become more positive.
