Objectice: Since few longitudinal studies were carried out, prospective effects of psychological job stress (PJS) on insomnia remain unclear. This prospective cohort study examined whether PJS caused persistence and future onset of insomnia among middle-aged workers. Methods: A total of 1,002 middle-aged (39-60 years old) Japanese workers were followed for two years. PJS was evaluated with the Demand-Control-Support (DCS) model and the Effort-Reward Imbalance (ERI) model. The relationship of high job strain, low work support, effort-reward imbalance, and over commitment to work at baseline with insomnia during the time of study was analyzed separately from insomnia at the baseline. Those who exhibited any of non-refreshing sleep, difficulty falling asleep, frequent sleep disruption, or early morning arousal for three or more nights per week during the previous month were defined as insomniacs. Results: After sex, age, illness, occupational conditions, and alcohol consumption at the baseline were adjusted, effort-reward imbalance [adjusted odds ratio (95% CI): 2.39 (1.12, 5.07)] and low work support [1.99 (1.17, 3.39)] at the baseline were significantly related to insomnia at the follow-up among those who were insomniac at the baseline (N = 292). Over commitment to work [1.76 (1.16, 2.67)] and high job strain [1.73 (1.07, 2.81)] at the baseline had a significant relationship with insomnia at the follow-up among those who were not insomniac at the baseline (N = 730). Conclusion: PJS evaluated with the DCS and ERI models predicts the persistence and future onset of insomnia among middle-aged workers.
