In past 10 years, there were many medications other than lithium, such as anticonvulsants and second generation antipsychotics were accepted and widely used to treat bipolar disorder. Some recent evidences have shown that the patterns of pharmacological treatment of bipolar disorder had changed. Objectives: 1. To study the pharmacological treatment of bipolar in the in-patient of Songklanagarind hospital during 1997-2007. 2 To compare pharmaciligical treatment patterns during last decade. 3. To study the treatment outcomes in the social and occupational adjustment at the 3 months after discharged from hospital. 4.To study the relapse rate at the 3 months after discharged from hospital. Method: Naturalistic, retrospective study of medical records of patients with bipolar disorder, manic or mixed episode whom admitted in psychiatric ward in 1997, 2002 and 2007. The demographic data, the diagnosis, last drug treatments of each admission were collected. We have evaluated the social and occupational adjustment and also recorded the relapse rate at the 3 months after discharged from hospital. Result: One hundred and fifty-two bipolar disorder patient records were reviewed, 139 had manic episode 13 had mixed episode, and 86 had psychotic features. 71.7% of the patients were treated with first generation antipsychotics, 55.3% were treated with anticonvulsants, 47.4% were treated with lithium. Only 19.7% of patients were treated with one drug group. Comparing the treatment patterns during past 10 years, We found that the lithium using rates were reduced from 68.9% in 1997 to 29.4% in 2007. The valproate using rates were increased from 11.5% to 67.7% and the second generation antipsychotics using rates were also increased from 1.6% to 55.9%, while the first generation antipsychotics using rates decreased from 78.7% to 50.0%at the same period. The mean number of psychotropic drugs were increased from 1.8 in 1997 to 2.6 in 2007. After 3 months followed up about 90% of patients had mild to moderate residual symptoms but can do the easier work. The relapse rate at 3 months was 18.4%. Conclusion: Prescription of lithium and first generation antipsychotics for the treatment of manic or mixed episode were decreased, while anticonvulsants and second generation antipsychotics using rates were marked increased. The relapse rate was rather high during 3 months follow up. Most of the patients who were not relapse had mild to moderate residual symptoms but they can do the easier work.
