Objective To study the utilization of the CES-D (Center for Epidemiologic Studies - Depression scale), Thai version, as a screen for depression in Thai adolescents. Method Subjects which consisted of 125 male adolescents aged 15-18 years filled out the CES-D and were evaluated with structured psychiatric interview by psychiatrists who were blind to the results of the CES-D. Results Subjects with interview validated depression had significantly elevated CES-D scores (mean = 25.6, SD=8.8 compared with nondepressed subjects (mean=15.4, SD=6.7), P‹10-6. Furthermore, the CES-D scores increased as the severity of depression increased. Using the Receiver Operating Characteristic Curve, a cut-off point of 22 produced the best overall screening characteristics (sensitivity=72%, specificity=85 % and accuracy=82%) Conclusion The results of this study indicate that the CES-D efficiently differentiates depressed from nondepressed adolescents and the optimal cutoff score in Thai adolescents is higher than the score previously recommended. Since the CES-D is an economical, easy to administer and readly analyzable instrument, it should be used as a screen and a supplant for clinical evaluation of depression in adolescents.
