Bipolar disorder is a common psychiatric illness. Lithium has been the drug of choices for the acute and maintenance treatment for a long time. However, many of bipolar disorder patients do not respond to lithium. The conventional antiepileptic drugs such as valproate and carbamazepine have been accepted as mood stabilizers. Mechanism of actions of antiepileptic drugs may be due to the enhanced inhibition of GABAergic system in the brain and/or inhibition of voltage-sensitive sodium channels. The newer antiepileptic drugs such as lamotrigine, gabapentin, topiramate, and tiagabine have shared these common actions and may have some mood stabilizing properties. Some controlled studies have shown that lamotrigine is effective in acute treatment in bipolar disorder. Slow adjusting dosage to avoid side effects is one of the limitation for using lamotrigine. Gabapentin is relatively safer and may be suitable to be used as an adjunct drug in refractory cases which have been reported by some open trials. Few early reports have shown that topiramate was effective in adjunctive treatment for manic episode. Tiagabine has some potential antimanic properties in few reported cases.
