Objective To evaluate the effectiveness of the Consciousness Transformation Program for Stress Management; and to develop a meditation-based stress reduction program. Method This is an experimental analytical pilot study, using the before–after study design with the same group of subjects. Some of the subjects were recruited from the patients who sought treatment at the Stress Reduction Clinic, Department of Psychiatry, while others were those who were interested in the study and volunteered to participate, from December 1996 to December 1999. The weekly training sessions took place for the period of eight weeks, consisting of (1) Breathing meditation : recognition; (2) Breathing meditation: awareness; (3) Body sensation awareness; (4) Emotional recognition and acceptance; (5) Emotional contemplation; (6) Awareness of thoughts and conditionings; (7) Absolute self-acceptance; and (8) Consciousness of love and understanding.The outcome was measured with the psychiatric condition scores elicited by the SCL-90 questionnaire conducted both before and after the treatment. Results Among 139 subjects (45 males and 94 females) , 46 subjects (33%) had adequate participation in the program, while 93 subjects (67%) dropped out before the completion of the study. It was found that the SCL-90 scores of the 28 subjects who completed the questionnaire statistically significantly reduced in 8 out of 9 items (p ‹ 0.05)--obsessive compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism. There was no statistically significant reduction only in the item concerning somatization. The overall percentage change was 23.6%. Finally, high dropouts occurred among the subjects who were students and those with the diagnosis of somatoform disorder, psychosomatic disorder, or borderline personality disorder Conclusions A Consciousness Transformation Program for Stress Management could be a useful adjunctive treatment for psychiatric patients so as to improve symptom profile in normal subjects. However, the patients who have prominent physical symptoms, lack psychological mindedness, and have a borderline personality disorder will not benefit from this treatment.
