Substance abuse has been shown to be associated with traffic accidents and injuries, The objective of this study was to determine the prevalence and types of substance abuse among traffic-injured patients attending emergency rooms of three hospitals (one in central Bangkok, one in an Bangkok suburb, and one in an expanding community). Thirty-five consecutive cases or their relatives of friends in each hospital were, after verbal consent, interviewed. Their blood and urine were collected for laboratory analysis for substances. Most injured cases were males with average age of 26.7 years. About 30% admitted history of alcohol use prior to the accidents and another 5% reported use of other substances, Results from chromatographic analysis revealed that alcohol was found in 39% of cases, morphine in 3%, amphetamines and met amphetamines in 5% and other substances in 10% of cases. Injured patients with detectable substances were more likely to be male and older than those without detectable level of the substances. There were suggestive evidences showing that detection of the substances might be associated with type of vehicles involved in the accidents and/or partial or complete loss of consciousness of the patients after the accidents. The high prevalence of substance abuse detected in this study indicates that all available measures be used to deal with the problem.
